Clubs may request a Certificate of Insurance (COI) for tryouts and practices by completing the 2019-20 Certificate of Insurance Request Form .
COI’s must be requested in a timely manner and can take up to ten (10) business days to process. The office will issue a COI to the Club Director. The CD must distribute it to any necessary recipients. There is no fee for this service. Facilities often have a preference of “Additional Insured” or “Evidence of Coverage only”, as well as General or Excess liability coverage. CD’s should confirm with each venue which options they need.
Practices
Clubs must request a certificate for specific dates and times at specific facilities for their practices (i.e. Mondays and Wednesdays from 6 to 10 pm at the KRVA Gymnasium). Each venue must be included in the request in order to be covered. Practices are not sanctioned until the Certificate of Insurance has been issued. There is no fee for this service.
Camps/Clinics/Leagues
Camps, clinics and leagues and their participants are not covered under USAV insurance unless the activity is sanctioned by KRVA, USA Volleyball, or another USAV Affiliated Region. Camps, clinics and league proposal(s) should be submitted. There is a $50 sanction fee for each proposal. Fee(s) must be paid to the KRVA Office BEFORE the activity is sanctioned. All players and coaches in attendance are required to have a current USAV membership.
